Holiday Craft Show Registration
Applications will be accepted June 1st – October 18th
Date: November 9, 2019
Time: 9:00am – 3:00pm
Set-up day before: November 8th at 8:30pm – 9:30pm. Please use side parking lot door
Set-up day: November 9th at 7:30am. Vendors must be ready to sell at 9:00am
Only 1 consultant allowed, example: Mark Kay, Thirty-one and Paparazzi
We ask that vendors start tearing down at 2:50pm. To reserve a booth space, you must fill out the registration form and turn it into the front office with payment. Vendors will be responsible for their own tables, chairs, extension cords and other displays. All money, tax and property are the sole responsibility of the vendor.
Please drop off or mail payment and registration at the FMS front office.

Vendors can drop off registration and payment starting August 13th.
Bring in current customers and new ones! We can e-mail you our Holiday Craft Show Flyer to spread the word. Or you can pick a flyer up at the FMS front office.
You will receive an e-mail before the Craft show with your booth number. All booths are in the GYM, Cafeteria or hallway.

Would you like to purchase lunch? We will be selling pizza with a drink for $5.00. This includes 2 pieces of pizza and a soda or water bottle. Concessions will also be open. If you would like to purchase lunch, please turn in money with this form and your registration form and booth payment.

By signing below, I understand and agree that Fruitport Middle School/PTA will not be held liable for accidents, theft, damage or loss. Refunds will not be given to cancellations on or after October 19, 2018 at 2:00pm. Refunds will not be given to no shows.
